VanEck Global Healthcare Leaders ETF (HLTH) — Review & Analysis

HLTH takes a more concentrated approach to global healthcare than IXJ. The fund tracks the MarketGrader Developed Markets (ex-Australia) Healthcare 50 Equal Weight Index — just 50 healthcare names, equally weighted rather than market-cap weighted. The equal-weight construction is the key differentiator. IXJ is dominated by mega-caps — Johnson & Johnson, Eli Lilly and UnitedHealth alone make up roughly 20% of its portfolio. HLTH's equal-weighting forces meaningful exposure to mid-cap healthcare names (medical devices, smaller pharma, healthcare services) that get crowded out of cap-weighted indices. That tilt has historically given equal-weight portfolios stronger upside in healthcare bull markets but more drawdown when megacap defensives lead.

The MarketGrader index also applies a quality screen on top of the equal-weight selection — companies are ranked on growth, profitability, cash flow and valuation before inclusion. HLTH is therefore part-factor, part-thematic — a quality-tilted healthcare basket rather than a passive sector tracker. This is a more active-feeling exposure than IXJ, but at 0.45% p.a. it's priced like a passive sector fund.

HLTH works for investors who want a smarter-beta approach to global healthcare rather than pure market-cap exposure. The smaller AUM means bid-ask spreads can be slightly wider than IXJ at execution, but liquidity is adequate for most retail trades.
